Abstract
Research Question: What are the guidelines, trends, and benefits of palliative care referrals used by primary care providers (PCPs) for common chronic conditions such as heart failure (HF), chronic obstructive pulmonary disease (COPD), Alzheimer’s disease, chronic kidney disease (CKD), and Parkinson’s disease (PD)?
Palliative Medicine is specialized medical care for people living with a serious illness focused on providing symptom relief and psychosocial support to improve quality of life for the patient, caregivers, and families. Primary palliative care may be provided by anyone. Secondary palliative care is provided by a specially-trained interdisciplinary team.
Currently, palliative care is mostly used by patients with cancer, followed by HF, coronary artery disease, COPD, dementia, then stroke
Benefits: symptom control, psychosocial support, caregiver support, disease education, continuation of curative treatment, improved healthcare experiences, decreased hospitalizations & ER visits, decreased healthcare expenses
Barriers: misconceptions, cultural beliefs, availability, coordination, training, reimbursement
