Utilizing Apolipoprotein B Measurement to Improve ASCVD Risk Assessment

Abstract

Each year in the United States, there are 2 million hospitalizations and 400,000 deaths due to atherosclerotic cardiovascular disease1. Total direct medical cost is expected to balloon to $818 billion by 2030 from $273 billion in the 2010s1.

Traditional cardiovascular risk assessment relies heavily on the lipid panel, most specifically LDL-C. LDL-C quantifies the cholesterol concentration within the LDL but does not quantify the lipoprotein itself.

Apolipoprotein B (ApoB) is a crucial structural protein found in all atherogenic lipoproteins and reflects total circulating atherogenic particle number.

Can ApoB become the new standard measurement to better assess ASCVD risk compared to the standard lipid panel, more specifically LDL-C?

This capstone project aims to discuss the shortcomings of the standard lipid panel, specifically LDL-C. Additionally, it will discuss the rationale of measuring ApoB, the contribution of ApoB in the process of ASCVD, and future implications of treatment of ASCVD given the utilization of measuring ApoB.

This capstone project is targeted towards Family Medicine and Cardiology healthcare professionals with the intention of showcasing a novel and evidence-based modality that could be utilized to better assess ASCVD risk in patients.

Family Medicine and Cardiology providers are often the first line of defense against ASCVD and integration of this new biomarker into clinical practice could equate to quicker risk stratification, improved management, and less burden on the healthcare system.
