Abstract
Occupational Therapy has a clear role to support high risk infants and their families during the transitional care period from the NICU to home, supporting participation in daily activities in the home and community, developmental monitoring, educating and coaching families, and enhancing the continuum of care for these infants while engaging in interdisciplinary and collaborative care.
Advocating for high-risk infants to support long-term neurodevelopmental monitoring for growth and development while assessing and providing recommendations for early intervention can help improve outcomes. These considerations require extensive materials to meet these supportive goals and networks through program development measures and considerations.
Purpose: Current high-risk infants discharged from MercyOne’s NICU are being referred to ChildServe. Program development materials for this program will support MercyOne in creating their own NICU follow-up clinic to support the continuum of care within the health system and increase opportunities for in-house referrals including sub-specialties and therapy.
