Abstract
On With Life at New Horizons is an adult day center primarily for individuals who have experienced a neurological diagnosis such as a stroke, brain injury, or dementia. The site also serves those with Down Syndrome, intellectual disability, or Autism.
- Rooted in recreational therapy with daily programming, pre-planned for all the individuals focused on the occupations of social participation and leisure activities
- A part of a larger organization, On With Life, who serves those with neurological diagnoses by providing rehabilitative and community-based services.
- Services include occupational therapy (OT), physical therapy (PT), speech therapy (SLP), neuropsychology, music therapy, recreational therapy, nursing, case management, and counseling services.
Needs of the site included:
- Rehabilitation services at the adult day center setting
- Individuals demonstrate long term physical and cognitive impairments impacting engagement and independence
- Program to support the integration of OT services at the site
- Including assessments, goal writing, interventions, and outcomes to address individualized goals for independence/ ability level

- Lack of identity/ feelings of purpose in life
