Abstract
Patient handoff definition: “The transfer of information, along with authority and responsibility, during transitions in care; to include opportunity to ask questions, clarify, and confirm. ”1
High-quality patient handoffs reduce medical errors, improve patient outcomes, and ensure continuity of care. When effective communication is utilized, the appropriate tests can be ordered and interpreted, the treatment plan is acknowledged and followed, and the entire healthcare team is on the same page for the maximum benefit of the patient.
Project Goals
Create a CME module to provide further education about patient handoff communication including barriers, training recommendations, and helpful mnemonics.
The target audience is all healthcare students and providers who provide patient care.
The future goals are to begin targeting some of these barriers, provide effective training to the entire healthcare team, and promote standardization of communication within hospital systems.
