Optimizing the Healthcare Transition for Children with Intellectual Disabilities to Adult Care

Abstract
Transitioning from pediatric to adult care is a vulnerable point in healthcare, particularly for individuals with intellectual disabilities (ID), a population that faces significant health disparities. While guidelines exist to ease this transition, a lack of standardized approaches remains. In 2023, the NIH recognized these health disparities and began funding research and programs, but more efforts are needed. People with IDs experience high rates of chronic illness, mental health disorders, emergency visits, and hospitalizations, highlighting the need for transition planning to ensure continuity of care and prevent negative outcomes. With over 1% of the population affected, providers across all specialties will encounter these patients, making this an important topic in medicine. Both personal and professional experiences with individuals with IDs inspired me to pursue my capstone project.

How can primary care providers optimize the healthcare transition to adult care for children and adolescents with intellectual disabilities?
